Your First Visit

To make your visit less time consuming please bring the following items with you to your visit.

  • Completed Forms
  • ID / Driver's License
  • Your Insurance Card
  • X-rays, MRI's, and any other studies
  • Office records from any other Physician (if previously treated)
  • Co-pay/Payment (if required)
  • Referral (if required by insurance company)
  • A parent or guardian if under the age of 18
  • If incapable of writing or understanding the English language must be accompanied by someone who can

Privacy Practices

Advanced Orthopedics and Sports Medicine, PLLC has policies and procedures to comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) Privacy Regulations. Advanced Orthopedics and Sports Medicine, PLLC has always supported and recognized our patients' rights to expect that their medical records and other information about their care will be kept confidential. The HIPAA privacy use and release of medical records, establishes appropriate safeguards that all health care providers must achieve to protect the privacy of health information. Violators of this policy will be held accountable with civil and criminal penalties.

One of the provisions of the HIPAA Privacy Regulations is that all health care providers distribute a "Notice of Privacy Practices". All patients will be given a notice when they register for services at Advanced Orthopedics and Sports Medicine, PLLC. Patients are not required to read the Notice, but we are asking that they acknowledge that they received the Notice by signing an acknowledgement statement.
